A comment here, and it's difficult to know if it represents one that is social, or rather one that relates to a rather fine interpretation of the rules for record keeping.

The 1888 birth registrations for Normanby only have been examined and while there is the odd one that does show the attendance of a physician, none of the returns to the township clerk were made by one. Every single birth recorded was either submitted by the father of the child (mostly) or a family member. Nowhere are mentioned the names of persons attending the birth who might have been family members (probable) or a midwife. And such returns can be found recorded by clerks in other townships or municipalities. Also, returns elsewhere can be found that were made by physicians who were not recorded as being attendant at the birth; they simply acted as informants.

Physicians were supposed to be supplied with forms that they could make such returns by mail, but these are not in evidence in Normanby in 1888.
