Elizabeth's surname, Burnley, as written on the registration might be almost anything. An index to be found on the website for the Brantford Expositor shows her name as Burnley. Also, the couples marriage record is to be found in the marriage records of Grace Anglican Church, Brantford, and this gives her family name as Burnley.

The registration appears to write George's family name as Shuttlesworth, but the marriage registration in the Grace Anglican Church records as well as census records, make it clear that the name was Shuttleworth. The Grace Anglican record also provides George's middle name as Howard.

Removal to Toronto Crematorium on Feb. 18th, 1939.

A transcription of Farringdon Burial Ground was done by the Brant County branch of the Ontario Genealogical Society in 2005. This shows George as being buried there with his wife, daughter, a brother and sister, and it's reasonable to believe that this is where he is buried. At first glance, that his remains were sent to Toronto Crematorium is counter-intuitive, but in 1939 such a service may not have been available in Brantford or other small towns in Ontario.
